We are hosting kindergarten registration on Friday, March 22nd from 2-4pm  

Complete the Student Registration Form 

Birth Certificate-Present school with original birth certificate.  A copy will be made for our records.

Immunization Record- Bring your record to be transcribed to the official school form.  Utah School Immunization Record

Proof of Residency  Provide two proofs of residency (must be dated within the last 60 days).

Guardianship Status Form- Will be asked to show valid I.D.
